To get the approval of a vendor for a BEAR form, schools will need to fax the entire
form. The company usually signs off on their verification page and faxes only that
back to you. You then substitute the faxed signed version for your original.
All forms should be sent in separate envelopes, Certified, Return Receipt Requested.

ADDITION to your BEAR form you must also complete their own "Data

include ATTN: E-Rate Group on the cover sheet).

As of 7/28, the following is the suggested VERIZON procedure for
Verizon Select when filing an ON LINE BEAR form: After posting the BEAR
form, send an email to
cindy.c.cunningham@verizon.com . For Local and Long Distance,
after the BEAR is posted, send an email to
Keona.J.Sims@verizon.com.
In both cases, let them know you have filed the electronic form and they
will let you know when the form has been certified.

E-Rate
Central has a good list of fax numbers for other vendors. The
same list also has SPIN numbers.
If you have the SPIN number for the company you want to contact, you
can use the
SLD search page to get the name of the contact person. The
contact person should be able to come up with the fax number. Use
the SPIN search NOT the name search. The SPIN search will yield a
more accurate number.
